Our Services
Initial Evaluation
Occupational therapy evaluation to include administering standardized testing, professional observations, collaborative interview with goal setting, followed by a comprehensive report and plan of care.
In Person Sessions
Together we will explore changes from the previous session, utilize therapeutic interventions to direct outcomes, and update home recommendations and plan of care as needed. These sessions are 45 minutes of 1:1 interaction, working on your specific plan of care.
Teletherapy
Working together through a secure and encrypted link, we will address the on-going steps in your individualized plan of care. This will include follow-up on recommendations from the previous session, as well as updating the home program based on client input and observed changes. Sessions will generally be 30 minutes in length.
Home Evaluation
Coming into your space, we can complete all the aspects of a typical evaluation, as well as addressing home safety concerns and making adaptive equipment recommendations. Once a plan of care has been established, if you decide to continue with further sessions, I am able to show you ways to use the things you already own as tools for meeting your goals.

Kimberly Eberly, OT
Hello and Welcome to my page! I am Kimberly Eberly (which I think is the best Dr Seuss name any OT could marry into!). I’ve been an occupational therapist for 28 years. In that time, I have supported adults and children from birth to 100+ years old in hospitals, nursing homes, schools, preschools, out-patient clinics, and clients’ homes, allowing me to see the full spectrum of human potential. The one thing that I have learned is that no matter the age or circumstances, we all want to be the best version of ourselves. I love helping individuals on their journey towards greater independence, confidence, and a healthy, balanced life! For that reason, I have now established my own private practice, pulling together all that I have learned so I can help people in the ways I know are best.
I have a professional interest in the intersection of visual rehabilitation, vestibular therapy, sensory modulation, developmental milestones, including reflexes, and sleep health. The way in which each person experiences each of these impacts how we move through and experience the world. Considering these, as well as other client-specific factors, we will create a plan of care together. Embedding strategies and habits into your daily life will help you meet your specific goals. And I am here to guide you with evidence-based interventions that work.
